REGISTRATION AND ENROLLMENT


THE FOLLOWING ARE REQUIRED FOR ENROLLMENT IN MITE, SQUIRT, PEE WEE, AND BANTAM ICE HOCKEY PROGRAMS FOR THE 2009-2010 SEASON
  • Completed Registration Application Form
  • Completed Player/Parent Rules and Regulations Form
  • Copy of the player's birth certificate
  • Cashier's check or money order for the required amount made payable to "NHT".
  • USA Hockey Registration and Confirmation Receipt

REGISTRATION PROCESS

To register please do the following:

1. Select the participant's division using the information in the table on the right.

2. Review the program description:

4a. New enrollees in the Mini-Mite and Mite Hockey Prep programs need to only need to bring a copy of the enrollee's birth certificate, the completed application form and payment. No appointment is necessary.

4b. New enrollees in the Mite, Squirt, Pee Wee and Bantam Teams must call the Riverbank Skating Complex at 212-694-3642 to make an appointment to interview with the Rink Director, Jean Darcel Michel or Rink Manager, Moises Rodriguez, at the Skating Rink Complex Office or walk in on one of the following open registration days:

Previously registered players do not need to submit a birth certificate and do not need approval to register. Simply fill out the application and submit the application with payment at the Cashier/Registration Window.

5. Again, if a child has never registered for a program at Riverbank before, please bring to the appointment or walk in open registration a copy of the participant's birth certificate in addition to the completed registration form.

6. For new participants of the the Riverbank Youth Ice Hockey Teams (mite, squirt, pee wee, and bantam) the Registration Application form must be approved by Rink Director, Jean Darcel Michel or Rink Manager, Moises Rodriguez. Complete the Registration Application form and let it be signed by the Rink Director or Rink Manager. Complete the Player/Parent Rules and Regulations Form and submit to rink personnel. After approval, the Registration Application form and birth certificate can be submitted for registration at the Cashier/Registration Window.

PAYMENT

7. Payment for program registration fees can be made by money order or cashier's check made payable to the "Natural Heritage Trust" or" NHT." Cash, personal checks and credit cards are not accepted for program registration fees.

The park registration fee and photo ID card can be paid by cash, money order, cashier's check or credit card.

You may submit registration materials and pay at the Cashier/Registration Window between 9 AM and 5 PM, Monday-Saturday, excluding holidays. On Wednesdays, the Cashier/Registration Window is open from 9 AM to 7 PM. Please come well before closing time. The Cashier/ Registration Window number is 212-694-3599.

When you submit the Registration Application Form and do not pay at that time, the form will be processed later and you will be mailed two (2) Global Sales Receipts. Upon receiving these receipts, check the receipt to ensure that enrollment is confirmed for the program selected. You can either 1) sign one and return it via mail along with your payment by money order or certified check or 2) you can bring one of the Global Sales Receipt to the Cashier/Registration Window in the Skating Complex and make payment there. Cash, personal checks or credit cards are not accepted for the hockey program registration fee.

We strongly recommend that you pay in person at the Cashier/Registration Window after you receive the receipt. You can also ask for expedited registration processing with the assistance of the Rink Director or Rink Manager so that the process is completed in one visit after the Registration Application is signed.


Activity Specific Registration Fees:


$110.00 - Mini-Mite and Mite Ice Hockey Prep Program

$175.00 - Mite/Squirt/Pee Wee/Bantam Programs

General Registration Fee: $4.00. First time registrants pay $14.00 ($4.00 registration fee and $10 for a photo ID card). Be sure to obtain a photo ID after payment is made.



USA HOCKEY REGISTRATION

8. Once enrolled in a ice hockey program, all participants 7 years and older are required to register online with USA Hockey for a fee of $33.00. This registration allows the participant to play in ice hockey games and competitions and provides basic insurance coverage. A subscription to USA Hockey magazine is included. USA HOCKEY REGISTRATION CONFIRMATION

9. After online registration is complete, a registration confirmation/receipt is displayed and one is emailed to the address you provided.

You must submit the confirmation receipt with barcode to the management personnel of the Riverbank State Park Skating Complex office as soon as possible. You can either print two copies of the screen, submit one by hand and keep the other one for your records or forward the e-mailed confirmation receipt by e-mail to:

We strongly recommend that you forward your confirmation receipt by e-mail and retain the original in your e-mail account.

You may register with USA Hockey before or after registration in one or more of the the Riverbank Youth Ice Hockey Programs.


IMPORTANT NOTE

Without exception, Riverbank State Park personnel will not allow any player who is not officially registered and enrolled in the one of the Riverbank Youth Ice Hockey Programs* to be on ice.

Please confirm enrollment prior to any ice hockey activity or event.

* Mini-mite, mite, squirt, pee wee or bantam program.
